Appointments are made either by phone on 01706 644717 or by email email@example.com. If we have your mobile or email details then a courtesy reminder of your appointment will be sent a few days before your appointment.
We also send out courtesy reminders by text, email or postcard one month before your dental examination is due.
We currently do not have an NHS contract for domiciliary visits, should you need a domiciliary visit please contact us and we can send out an appropriate referral.
Please contact us as soon as possible if you wish to cancel an appointment, this can be done by telephone on 01706 644717 where a message can be left if necessary or by email firstname.lastname@example.org. Please give 24 hours notice if at all possible.
Please note that if you either fail to attend or cancel with less than 24hours notice on 2 occasions you may longer be offered NHS treatment. If it is a private appointment then a fee may be charged.
